Choose who is on shift
The room and the hour stay fixed. Who is standing beside the bed changes the whole tone.
Why the role does the work
She has permission to be close to you and a professional manner to hide behind, which gives the scene a built-in tension between what she is supposed to be doing and what she is actually doing. Teasing works well here because she can always retreat behind the job.
